New Patients

New patients should arrive 20-30 minutes prior to their appointment for completion of their registration packet. It may also printed from here, completed at home and brought to appointment.

Please bring the following items with you to your first visit:

Insurance Cards
Photo ID
All medications, including over-the-counter. It is very important that you bring the actual medication bottles.

Treat this as a normal day. Eat, drink and take all of your regularly scheduled medications. Be prepared to give the nurse a urine sample when you arrive if your appointment is at our main Rockford location.

Follow-Up Patients

Please arrive 10 minutes prior to your scheduled appointment time. Bring a photo ID, insurance cards and bottles of medication. Please notify the receptionist of any changes in your address, phone number or insurance information upon check-in.
