August 1st, 2006 DaVitaCare has joined RNA and has changed it's name to CKDcare. We are very excited and our patients will notice only minor changes in service.

We will still provide a multitude of support services which include Medical Nutritional Therapy(MNT), anemia management and dialysis decision support and coordination.

DaVitaCare and RNA Change is in the air... Patients currently participating in CKDcare may experience some minor changes in service delivery. As of August 1st, lab tests will be drawn and processed by Rockford Health Physicians lab (or by the lab of your choice), rather than in the CKDcare clinic. We expect that most lab results will be available a little sooner due to this change. It will, however, require tha patients make an additional stop in the clinic lab. We will provide lab services on site at the Roxbury location after we move. Patients will also notice a difference in their medical bills. As CKDcare will be a part of Rockford Nephrology Associates (RNA), billing will be provided through RNA, rather than DaVita. In addition, the CKDcare clinic is fortunate to have two RNA nurse practitioners and a medical assistant joining our CKDcare family to assist in providing care to our growing patient population. We are happy to welcome Yvonne Schoonover APN/CNP, Kathy Capriola APN/CNS, and Angela Meskimen, MA! CKDcare moves to Roxbury Road The chronic kidney disease program, also known as CKDcare, is happy to announce that they moved to a new location in September and opened on September 19th of this year. The new Rockford Nephrology Associates building is located at 612 Roxbury Road, Rockford, Illinois. We are very excited about our new space and feel it provides a pleasant and convenient new environment for our patients. KEY-Kidney Education and You Have you been told by your doctor you may need to be on dialysis? Are you a candidate for a kidney transplant? If you answered yes to either of these questions, KEY-Kidney Education and You, may be able to answer your questions about treatment choices available for treatment of your chronic kidney disease. An educational program is available at the Rockford Nephrology Associates and CKDcare office monthly to discuss kidney disease, treatment choices, financial concerns and dietary recommendations related to chronic kidney disease. KEY-Kidney Education and You programs are offered the 2nd Tuesday of each month at 9:00am and 1:30pm. Registration is required and anyone is welcome to participate in the program.
